Blackfoot Hospice Care
What is hospice?
Hospice care is for those with a terminal illness that no longer wish to receive treatment. Hospice care is centered on making the patient feel comfortable, and providing support to both patient and family members during this unique stage. The Solace Blackfoot hospice care team is comprised of doctors, nurses, aides, emotional and spiritual support, and more. All of the staff are specifically trained for the end of life stage, and are sensitive and understanding of the difficult decisions patients and families make to reduce or end treatment.
Our hospice service includes a personalized care plan. We want to serve you in the way that best fits your needs and wants. The care plan will include things like which medications to continue or discontinue, how often and what level of staffing visits you require. It is important for us to make sure we are on the same page and have the same goals. A personalized plan helps us understand our role in your end of life care. The plan will include physical as well as emotional needs and support.
Who can receive hospice care?
In order to qualify for hospice services, the patient must have a prognosis of six months or less with an incurable disease. Some choose hospice care soon after a terminal diagnosis. Others choose to transition into hospice care when previous treatments are no longer working, or pain and suffering outweighs the desire to continue. Age has no relevance in someone’s ability to qualify for hospice care if they meet the above mentioned requirements. If you or a loved one is considering hospice care you must understand that this decision, although not permanent, often leads to a swift decline.
While there are a variety of illnesses that lead people to seek hospice care, some of the most common conditions include:
- Cancer
- Liver disease
- Heart disease
- HIV
- Stroke
- ALS
Will insurance cover hospice care?
The short answer is yes. If you are an elderly individual who qualifies for Medicare, Medicare Part A will cover hospice services. If you do not qualify for Medicare, most private insurances as well as government plans such as Medicaid will approve hospice care. If none of these are an option for you, contact our office to discuss private pay options.
Blackfoot Hospice Care Services
We offer a wide variety of services at many different locations. We want what is convenient and comfortable for you. We can provide medical equipment as well as hands-on staffing and specific education. Whether you need full-time hospice care, or respite care and access to services while caring for your own loved one, Solace Hospice can help. The following are brief descriptions of the different roles of specific team members.
Doctors
We have two doctors that work together with the patient’s primary care doctor to navigate this time and provide the best approach to end of life care. Dr. Bryan Hammar or Dr. Brady Martin will oversee the hospice process. They can suggest treatment plans, and collaborate with other staff members to create a cohesive team.
Nurses
Part of effective hospice care is having access to nursing staff 24 hours a day, 7 days a week. Our nurses are always available and provide a wide range of services such as respite care, wound care, and medication management. Even with in-home hospice, our nursing staff can serve in the same way as they would in a hospital setting. Nurses are the key to coordinating care between all other staff.
Nurse Aides
Activities of daily living such as getting dressed and personal care and hygiene are the main focus of the nurses aides. They can also help teach family members how to assist with these routines. If requested, nurse aides can also help with meal preparation. Their ultimate goal is to create a comfortable environment for the patient and take care of the hygienic tasks that become increasingly difficult to do each day.
Support Staff
There is so much more than just physical health that needs to be incorporated into a hospice personal care plan. Approaching this unique phase of life requires attention to all aspects of health for the patient and those close to them during this process. Our support staff team includes a social worker, spiritual care coordinator, bereavement, and respite care staff.
